Procedures Admission decisions are made when the entire application process has been completed. The following information is required to complete the file for evaluation. A. Copy of Baptismal Certificate (Catholics) 4. Parish Verification Form 5. Social Security Card 6. Birth Certificate 7. Georgia Health Screening Certificate (EED) 8. Georgia Immunization Record Form 9. Financial Assistance Application (optional) B. Previous School Records: 1. A complete and current Academic Transcript 2. Educational / Achievement Test Scores C. Testing: Our Lady of Lourdes Catholic School utilizes standardized assessment instruments to determine proficiency in math and communicative arts. The test is used to determine appropriate grade level assignment or indicate the need for additional help to reach grade-level expectations. Appointments for testing are scheduled during open enrollment, by request and prior to the start of each school year in August. Contact the office for an appointment or for further information.
