2. Application and Testing Fees for new students - $50.00

of Baptismal Certificate (Catholics)

Georgia Health Screening Certificate (EED)

Georgia Immunization Record Form
9.
Financial Assistance Application (optional)
B. Previous School
Records:
1.
A complete and current Academic Transcript
2. Educational
/ Achievement Test Scores
C. Testing:
Our Lady of
Lourdes Catholic School utilizes
standardized
instrument to determine proficiency
in Math and
Communicative Arts. The test is
used to
determine appropriate grade level assignment
or indicate
the need for additional help to reach
grade-level
expectations.
Testing times are
scheduled
during open enrollment times, by
requested
appointment and in August prior to
the start of
the school year.
